The Jacob Center offers a variety of programs for youth. Our highly trained staff strive to provide the best for families, and we look forward to working with you!   • Family Visitor Program

    • The Family Visitor Program provides staff members to complete in-home visits with families identified by Larimer County Department of Human Services (LCDHS).

  • Supervised Visitation

    • Provides visitation staff to coordinate and monitor visits between children placed in out of home care and their parents or family.

  • Trauma Treatment Coordination (TTC)

    • We incorporate evidence-based trauma informed practice into all of our services, however offer specific programming that is intended to assess trauma impacts, offer recommendations, and implementation of resiliency plans.

  • TBRIĀ® Caregiver Training

    • TBRIĀ® - Trust Based Relational InterventionĀ® is a holistic, attachment-based, trauma-informed intervention that is designed to meet the complex needs of vulnerable children.

  • Youth and Family Coaching

    • Consisting of both one-on-one, and group mentoring, coaching aims to provide individualized, short term, goal focused support services to youth and families both in and out of home.
